| IMMUNIZATION INFORMATION |
Florida law requires that every child entering a public or nonpublic school in Florida be inoculated in compliance with the schedule found here. Mandatory inoculations have caused some debate due to reported negative reactions from inoculations, especially among young children. The state law does allow a parent to make an independent vaccination decision for themselves and their children, but only through an official letter of exemption. Without a letter of exemption a child may not be enrolled who does not comply with the mandated inoculation schedule. For more information, please consult your physician.
1. No student will be permitted to begin classes without the proper health records being on file in the nurse's office.
2. ALL children in the preprimary program (PPI through 1st grade) must submit the following:
• The HRS 3040 Medical Examination Certificate dated within a year prior to August 2005. • Certificate of Immunization (HRS 680 A or B form) appropriate for child's age must be up-to-date.
3. ALL children in Preprimary, Kindergarten through 4th grades are required to have the Varicella vaccine (Chicken Pox) unless he/she has already had chicken pox, then disregard.
4. ALL children entering Kindergarten-6th grade must have a second dose of measles vaccine, preferably MMR, prior to the first day of school. (Please disregard if your child had a 2nd MMR or 2nd dose of measles vaccine last year.)
5. ALL children entering Preprimary through 6th grades must have completed the Hepatitis B vaccine series.
6. ALL children entering 7th-12th grades must have completed the following:
• Hepatitis B vaccine series • Second dose of measles vaccine, preferably MMR • Tetanus, Diptheria booster
7. All NEW incoming students must have the HRS 3040 Medical Examination Certificate and a Certificate of Immunization HRS 680 A forms.
8. Students transferring from other states must submit Florida Certificate of Immunization (Dept. of Health Form 680).
EXACT DATES OF IMMUNIZATION ARE REQUIRED. All vaccine dates are to include the month, day and year.
DPT (Diptheria, Pertussis, Tetanus) 5 doses required OPV (Polio) 4 doses required MMR (Measles, Mumps, Rubella) 1 dose of individual vaccine or MMR combined form at age 12 months or older and a second dose prior to entering Kindergarten through 12th grade. HEPATITIS B vaccine series for Preprimary through 12th grades. VARICELLA vaccine is required for ALL preschoolers, Kindergartners through 3rd graders unless the disease was previously contracted.
EXEMPTIONS: Temporary (DOH 680B) or permanent (DOH 680C) is permissible by law if a competent medical authority certified that the child should be exempt. Religious (DOH 681) is issued only by county health departments.
9. If your child requires medication (prescription or over-the-counter, including Tylenol) during the school day, in order for the school nurse to dispense medication, the PHYSICIAN'S AUTHORIZATION TO DISPENSE MEDICINE form enclosed must be submitted to the nurse in advance and must be signed by a physician and parent or guardian. Please complete the form in full, including amount of medication to be given and the times. |
