| Download PDF for Transcript Request Form | In order to request transcripts, please do the following:
1. Download the Transcript Request Form. Complete all sections of the Transcript Request Form (Make sure you sign and date it.) 2. Fax or mail the completed form to the attention of Registrar, Student Services. 3. Send $2 payment for each transcript requested. The payment may be sent by a check payable to Westminster AcademyŽ or in cash. Both should be mailed to the attention of Registrar, Student Services. The fee is waived for current students and for students who graduated or withdrew during the previous school year. 4. Once authorization to release the records has been received from the Finance Office and the payment has been received, then the records will be sent to the place(s) indicated on the Transcript Request Form. 5.
