Health Record Requirements School Year
Florida law requires that every child entering a public or nonpublic school in Florida be inoculated in compliance with the schedule found here. Mandatory inoculations have caused some debate due to reported negative reactions from inoculations, especially among young children. The state law does allow a parent to make an independent vaccination decision for themselves and their children, but only through an official letter of exemption. Without a letter of exemption a child may not be enrolled who does not comply with the mandated inoculation schedule. For more information, please consult your physician.
Preprimary Students:
1. DOH 3040 - Student Health Examination or Statement of Good Health, with TB screening date and result, signed by a physician and dated within one year of the first day of school.
2. DOH 680 Part A Florida Certificate of Immunization or DOH 680 Part B - Temporary Medical Exemption for preschool children who have incomplete immunizations on 680 Part A-1 or A-2.
3. Hepatites B vaccine series and Varicella vaccine or documentation of history of Varicella disease (Chicken Pox)
Kindergarten - 6th grade:
1. DOH 3040 - Student Health Examination or Statement of Good Health, with TB screening date and result, signed by a physician and dated within one year of the first day of school.
2. DOH 680 Part A-1 Florida Certificate of Immunization with proof of 2 doses of Measles vaccine, preferably MMR (Measles, Mumps, & Rubella).
3. Hepatitis B vaccine series for all students entering kindergarten through 6th grade.
4. Varicella vaccine or documentation of history of Varicella disease (Chicken Pox) for all students entering kindergarten through 3rd grade.
7th-12grade:
1. DOH 3040 - Student Health Examination or Statement of Good Health, with TB screening date and result, signed by a physician and dated with one year of the first day of school.
2. DOH 680 Part A-2 Florida Certificate of Immunization with proof of:
Hepatitis B vaccine series 2nd booster dose of Measles, preferably MMR (Measles, Mumps, & Rubella) Tetanus Diptheria booster dose
For Exemptions:
Permanent Medical Exemption - DOH 680 Part C
A child who is not fully immunized, but who for medical reasons cannot receive one or more vaccines, may be issued Permanent Medical Exemption provided the physician states in writing the medical reasons based upon valid clinical reasoning or evidence of demonstrating the need for a permanent exemption.
Religious Exemption from Immunization - DOH 681
Florida adminstrative rules allow exemption from immunization for religious reasons. DOH 681 is issued only county health departments..
For Out of State Families:
Students transferring from out of state are given 30 days to transfer immunization records ot a DOH 680 A Florida Certificate of Immunization. This must be accomplished at a physician's office in Florida or at a branch of a Florida Public Health Unit. Vaccines are available free of charge at our local Broward County Public Health Unit.
